Job summary

UnitedHealth Group seeks a Field Based HSS Coordinator near Eagle Pass, TX. You will perform home-based assessments, coordinate services, and travel up to 75% to Medicaid members, often within 100 miles of your location.

Strong MS Office skills, a valid driver’s license, and reliable internet are required. This role supports an Integrated Care Model, with an emphasis on timely documentation, advocacy for best care, and field-based collaboration.

Qualifications

• Must meet one of the listed educational/experience combinations.
• Proficient with MS Word, Excel and Outlook.
• High speed internet at residence.
• Reliable transportation with US driver’s license; ability to travel up to 75%.

Responsibilities

• Complete face-to-face assessments in members' homes to address unmet needs
• Travel up to 75% of the time, including travel up to 100 miles from the assigned work location
• Meet documentation and turnaround times based on state contractual requirements
• Time management and organizational skills
• Coordinating members' services
• Ability to create, edit, save and send documents, spreadsheets and emails
• Reside within commutable distance of assigned duties
• Works on Service Plan
