Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician (NP/PA) to provide and coordinate primary care for seniors in living communities across Arizona. You will conduct exams, administer vaccines, and manage chronic conditions with a focus on timely, appropriate care.

You will collaborate with care management and PCPs, document visits, and participate in community education events. A field-based role involves travel across Tucson and Green Valley, with ongoing professional development

Qualifications

• Master’s degree in Nurse Practitioner or PA program.
• Arizona NP license or PA-C in good standing.
• Wound Care and Palliative Care certificates are a plus.

Responsibilities

• Provide and coordinate primary care to SCAN members in living communities.
• Conduct physical exams, vaccines, and tests; triage to avoid unnecessary hospital visits.
• Collaborate with case management and Primary Care Physicians to ensure timely care.
• Document visits per established protocols and support care planning.
• Engage with communities and participate in events to educate members.
