Job summary
Pinnacle Family Services, part of Health Connect America, invites you to join our mission-driven team delivering Family Centered Treatment in home and community settings. You’ll work with children, adolescents, and families to implement structured, evidence-based care and coordinate with medical and school partners to ensure holistic support.
Certification in FCT is required within one year, with structured trainings and supportive leadership.
Qualifications
- Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
- Master's in human services field + 1 year experience or
- Bachelor's in human services field + 2 years experience or
- Bachelor's in non-human services field + 4 years experience
Responsibilities
- Deliver Family Centered Treatment services primarily in home and community settings.
- Guide families through the four phases of the FCT model and set goals.
- Coordinate with medical, behavioral health, school, and community partners.
- Develop, implement, and refine individualized Service Plans with families.
- Connect families to community resources and aftercare supports.
- Maintain timely documentation in the EHR.
- Participate in Quality Assurance activities.
