Pacific Health Group is seeking a Lead Care Coordinator in California. This field-based, hybrid role supports members in the community by coordinating medical, behavioral health, and social services, with frequent in-person visits and strong emphasis on housing, transportation, and access to care.

Responsibilities include managing a 60–70 member caseload, documenting in real-time, and collaborating with community partners. Travel up to 60–70% within the county is expected.

Qualifications

• Residency in hiring county.
• 3–5 years in case management, social services, or healthcare (preferred).
• Medi-Cal, CalAIM, or ECM familiarity (preferred).
• Experience with healthcare systems and community resources is a plus.
• Excellent communication, organization, and time management.
• Proficiency with documentation systems and technology.
• Must travel up to 60–70% within the county.

Responsabilités

• Manage a caseload of approximately 60–70 members.
• Conduct 3–5 in-person visits per day (homes, shelters, community settings).
• Spend 60–70% of time in the field.
• Coordinate care across medical, behavioral health, and community services.
• Document in real-time using internal systems.
