APHealth is seeking an experienced Regional Program Coordinator to lead ECM, CS, and CHW services in Northern California. This field-based leadership role supervises Care Managers, manages program performance, ensures compliance, and maintains a small caseload of about 20 members.

Candidates must reside in Northern California, be willing to travel extensively, and align with CalAIM and HIPAA requirements. The role offers remote work with field visits and opportunities to advance regional growth.

Qualifikationen

• Bachelor's degree in Social Work, Public Health, Human Services, Psychology, Nursing, or related field.
• At least five years of experience in care management, healthcare, behavioral health, case management, or social services.
• At least three years of supervisory or management experience.
• Knowledge of CalAIM, Enhanced Care Management, and Community Supports programs.
• Strong leadership, communication, organization, and problem-solving skills.
• Experience working with vulnerable populations or individuals with complex needs.
• Proficiency with electronic health records, care management platforms, and performance reporting systems.

Aufgaben

• Supervise, coach, mentor, and support Care Managers.
• Conduct performance reviews and staff development.
• Monitor productivity, documentation quality, member engagement, and outcomes.
• Lead team meetings, case reviews, and quality improvement activities.
• Coordinate day-to-day regional operations for ECM, CS, and CHW programs.
• Ensure compliance with CalAIM, DHCS, HIPAA, and contract requirements.
• Maintain a direct caseload of about 20 members.
• Represent APHealth in the assigned region and build partner relationships.
• Travel frequently and perform in-person member visits as needed.
