OneWell Health Care in Evans, GA seeks a Direct Support Professional (DSP)/Caregiver to deliver compassionate, person-centered care to individuals with IDD and Autism in home and community settings. You will assist with daily living, health monitoring, and behavior support while promoting independence and dignity.

The role is field-based and part-time, requiring adherence to ISP/IPP plans and accurate EVV documentation. Training and certifications such as CNA/HHA/PCA/PCT may be required.

Formación

• English proficiency in spoken and written form is required.
• High school diploma or equivalent is required.
• Candidate must possess at least one active certification: CNA, HHA, PCA, or PCT.

Responsabilidades

• Provide direct care and support to individuals with IDD and Autism in home and community settings.
• Assist with activities of daily living and health monitoring.
• Document services daily and maintain EVV records.
• Follow ISP/IPP and care plans under supervision.
• Support safety, independence, and community participation.
