Responsibilities

Outreach to community members and connect them to care resources.
Provide health education and support to members and care team.
Conduct non-clinical assessments and refer to appropriate care/services.
Perform home visits and community outreach to locate hard-to-reach individuals/families.
Assist with telephonic outreach and in-person communication as needed.
Document and maintain community resources per health plan policies.
