Job summary

Centene's Latham, NY office is seeking an in-office Community Resources Coordinator to help connect members to community resources and support the care management journey with health education.

The role processes all faxes the same day, coordinates with Utilization Management, and assists nurses and social workers with outreach, home safety and non-clinical assessments, while complying with policies and standards.

Qualifications

• High School diploma or GED required.
• 1–2 years related experience preferred.

Responsibilities

• Support members in connecting to community and care resources.
• Coordinate community outreach resources and promote awareness of care/services.
• Assist members with inquiries and provide health education.
• Support member correspondence and educational materials for care coordination.
• Document and maintain community resources per health plan policies.
• Assist clinical team (nurses and social workers) with outreach and guidance.
• Conduct non-clinical assessments to refer to appropriate services and gather information for providers.
• Perform telephonic and in-person outreach to locate hard-to-reach individuals and families.
