Centene Corporation is seeking a field-based outreach specialist in North Carolina to connect members with community and care resources and support the care management journey.

The role requires a high school diploma or GED and 1–2 years of related experience, with a schedule Monday–Friday, 8:00 AM–5:00 PM, and possible evening/weekend hours. Candidates must reside in listed NC counties and be computer savvy with strong communication and assessment skills.

Responsibilities

• Provides support to members to connect them to known community and care resources in a cost-effective manner.
• Supports the coordination of community outreach resources available to members and promotes awareness of care/services.
• Assists with community and care resource inquiries and opportunities available to members.
• Supports member-related correspondence and educational materials to facilitate community connections.
• Documents and maintains all community resources per health plan requirements.
• Assists the clinical team with outreach, community education, and member support.
• Conducts non-clinical health assessments to refer members to appropriate care/services.
• Conducts non-medical assessments such as home safety and transportation to refer to appropriate care/services.
• Performs telephonic and/or in-person outreach to locate hard-to-reach individuals and families.
• May make visits to homes and community organizations.
• Shows working knowledge of Social Determinants of Health (SDOH).
• Performs other duties as assigned.
