Job summary

Qualifications

• Bachelor's degree required.
• 2+ years of experience in community health outreach or related field.
• CHW Massachusetts License preferred.
• Experience with Social Determinants of Health (SDOH) preferred.
• Experience helping patients apply for state/federal programs and advocating for patients.

Responsibilities

• Provide advanced health education, outreach, and care coordination to individuals and families.
• Work with healthcare providers to ensure appropriate care and support services.
• Conduct in-depth assessments of patient needs, goals, and barriers.
• Develop and implement care plans for patients with complex health needs.
• Assist patients in navigating the healthcare system and accessing services.
