Job summary

Trinity Health Of New England is seeking a Community Health Worker to support care management within our Clinically Integrated Network. This full-time, day-shift role focuses on coordinating patient records and facilitating access to community resources across western Massachusetts and Connecticut.

You will educate patients using knowledge of medical conditions, nutrition, and diet, while collaborating with partners like VNA agencies and DME providers to improve health outcomes.

Qualifications

• High school diploma required.
• Medical Assistant Certificate preferred; Associates degree or equivalent medical experience preferred.
• Certified Application Counselor for MassHealth or willing/able to complete trainings to support patients in demonstrating eligibility requirements.
• Minimum of five years’ work experience addressing Behavioral Health (BH) and Health Related Social Needs (HRSN) of children, youth, and families in a health care setting and/or possessing specialized training or certification.
• Minimum of two years' experience within a healthcare related organization (e.g. health system, hospital, clinic, provider practice, health insurance company, etc.).
• Bilingual candidates are encouraged to apply.

Responsibilities

• Mail educational materials and letters, schedule patient appointments, locate and obtain outside patient records
• Collaborate with community partners, such as VNA agencies, caregiver programs, and DME providers
• Use general knowledge of medical conditions, nutrition, and diet to educate patients
• Assist in locating and updating patient clinical records
• Facilitate patient and caregiver access to community resources programs, Meals on Wheels, senior centers, chore services, etc
