Job summary

Care New England Health System seeks an Integrated Behavioral Health Community Health Worker to join the care team in Rhode Island. You will engage children, families, and adults, provide education, and help coordinate behavioral and developmental health services at the point of care in the primary care clinic.

Under supervision of a Behavioral Health Clinician, you will assess needs, connect families with community resources, and support adherence to care plans in a culturally competent manner.

Qualifications

• Bachelor’s or associate degree preferred in social work, human services or related field.
• Excellent communication skills and ability to work with diverse populations.
• Bilingual abilities are highly encouraged to apply.

Responsibilities

• Engage with patients and caregivers to build trust and promote care adherence.
• Provide health education and support to address developmental and behavioral health needs.
• Coordinate referrals and connect families to community resources.
• Document case notes and maintain patient confidentiality.
• Collaborate with interdisciplinary team and participate in trainings.
