Job summary

Qualifications

• Proficiency with Microsoft Office or applicable software.
• Excellent verbal and written communication skills.

Responsibilities

• Advocate for members and connect them with health care resources.
• Perform non-clinical paraprofessional duties in the field (home visits, shelters, provider offices).
• Help members navigate and maximize health plan benefits and arrange transportation or appointments.
• Assist members in accessing social services and community resources.
• Outreach to reconnect disconnected members with special needs.
