Job summary

Saint Luke’s Health System is seeking a Community Health Worker to serve the North Region Primary Care Clinics and surrounding areas. The WHP CHW will travel across the coverage area to support at‑risk patients, coach them in managing their health, and help navigate the health care and social service systems.

The role involves home visits, attending medical appointments, connecting patients with resources, and documenting all interactions in a timely, accurate manner.

Qualifications

• CHW certification within 1 year of hire.
• Experience serving underserved and diverse populations in community settings.
• Reliable and accessible transportation with current insurance.
• Ability to relate to diverse groups and communicate effectively with patients and partners.

Responsibilities

• Support at-risk patients to manage their health and navigate health and social services.
• Conduct home visits, accompany patients to medical appointments, and link to community resources.
• Complete required documentation accurately and timely in approved formats.
