Qualifications

• Ability to travel 70% of the time; reliable transportation.
• 3–5 years' relevant work experience.
• 1–2 years' experience providing clinical support services.
• 1 year community outreach experience in LA.
• 1 year working with clients with clinical needs.
• Proficiency with MS Office Suite.
• Availability to work outside core hours and evenings.
• Must be able to work 2 evenings weekly.

Responsibilities

• Travel to member homes up to 70% of the time.
• Educate providers on social determinants of health.
• Build relationships with members and care teams.
• Assist with care plan implementation and referrals.
• Help identify socio-economic barriers and link to services.
• Maintain member records in EHR and prepare reports.
• Enter data and complete program documentation promptly.
• Accompany members to appointments when needed.
