Job summary

CVS Health is seeking a Community Health Worker to support Aetna members in Louisiana's Orleans, St. Bernard, Jefferson, and Plaquemines parishes. The role bridges community and healthcare systems through outreach, education, and care coordination.

The position requires 70% in-state travel, in-home visits, and documentation in electronic health records. It’s a full-time role offering benefits and opportunities to improve health outcomes.

Qualifications

• Travel 70% of the time; reliable transportation.
• 3-5 years' relevant work experience
• 1-2 years' experience providing clinical support services
• 1 years of community outreach experience and knowledge of LA community resources
• 1 years' experience working with clients with clinical needs (physical and/or behavioral health)
• Demonstrated proficiency with a personal computer, keyboard navigation, and MS Office Suite applications
• Flexibility to work outside of core business hours Monday-Friday, 8am-5pm
• Must have ability to work 2 days weekly until 9pm local time.

Responsibilities

• Conduct home visits with members to reinforce care plans and improve access to care.
• Educate professionals on social determinants of health and care coordination.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Assist with care plan implementation and health education.
• Identify socio-economic issues affecting health and develop management plans.
• Assist with scheduling appointments and navigating social services.
• Support medication adherence and disease self-management.
• Accompany members to appointments and maintain records in the EHR.
