Job summary

Qualifications

• Four or more years of related experience and/or training.
• Medical experience strongly preferred.
• Experience with diverse populations, including underserved communities.
• Willingness to drive with a clean driving record.

Responsibilities

• Provide field-based care coordination to address behavioral, physical, and social needs of assigned clients.
• Assess social, financial, and health needs of individuals within the community.
• Connect clients with housing, food, healthcare, transportation, and mental health resources.
• Develop and implement individualized care plans with clients.
• Travel throughout New Orleans and Orleans Parish for in-home support.

Skills

Excellent communication skills

Proficiency in Microsoft Office Suite

Willingness to drive with a clean driv

Education

Associate’s degree in Social Work, Sociology, Psychology, Counseling, or related field

Active LPN/RN licensure, EMT, or Paramedic certification

Current BLS certification

Current and clear driver’s license
