Qualifications

• Associate’s degree or active LPN/RN licensure required.
• Current BLS certification and TB testing required.

Responsibilities

• Provide field-based care coordination addressing behavioral, physical, and social needs.
• Assess social, financial, and health needs of individuals and families.
• Connect clients with housing, food, healthcare, transportation, and mental health resources.
• Develop individualized care plans in collaboration with clients.
• Maintain HIPAA-compliant EMR documentation and follow confidentiality guidelines.
• Travel throughout New Orleans and Orleans Parish for in-home visits.
