Adobe Population Health (APH) is seeking a Community Care Case Manager to coordinate and manage community-based care for members in Albuquerque, NM. The role requires travel throughout Bernalillo County to support individuals with comprehensive care planning and services.

Qualifications include a Bachelor’s degree, CPR, and a valid driver’s license. The position emphasizes collaboration with health plans, providers, and social services to improve health outcomes and independence.

Qualifications

• Two (2) years of relevant social work or healthcare-related experience preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a team and with diverse resources.

Responsibilities

• Coordinate care and ensure services align with 42 C.F.R. § 438.208 and contract requirements.
• Perform Health Risk Assessments per NM guidelines to determine need for comprehensive assessments.
• Classify members and develop Comprehensive Care Plans (CCPs).
• Deliver ongoing care coordination and respond to member needs.
• Collaborate with health and community resources to address member needs.
