Job summary
Vitality Hospice & Palliative Care is seeking a compassionate Certified Nursing Assistant (CNA) for a full-time field-based role. You will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during the end-of-life journey.
The role offers typical hours 7:00 AM – 3:00 PM, Monday–Friday, with occasional evenings/weekends and mileage reimbursement.
Qualifications
- Active CNA certification required.
- Valid driver’s license, reliable vehicle, and current auto insurance.
- Ability to work independently in the field.
- Strong communication, time-management, and organizational skills.
- Compassionate, dependable, and patient-focused.
- Experience with hospice, palliative care, or chronic illness preferred (not required).
Responsibilities
- Assist patients with mobility, transfers, and ambulation.
- Provide personal care including bathing, grooming, dressing, and feeding.
- Offer companionship and emotional support to patients and families.
- Perform light housekeeping tasks related to patient care.
- Accurately document care provided in a timely manner; follow individualized care plans and hospice standards.
- Support a culture of compassion, respect, and patient-centered care.
