Job summary

Vitality Hospice & Palliative Care is seeking a compassionate Certified Nursing Assistant (CNA) to join the field-based interdisciplinary team. You will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during end-of-life care.

The role is full-time with standard hours 7:00 AM – 3:00 PM, local travel, mileage reimbursement, and immediate hiring.

Qualifications

• Active CNA certification required.
• Valid driver’s license and reliable vehicle.
• Ability to work independently in the field.
• Strong communication, time-management, and organizational skills.
• Experience with hospice or palliative care preferred (not required).

Responsibilities

• Assist patients with mobility, transfers, and ambulation.
• Provide personal care including bathing, grooming, dressing, and feeding.
• Offer companionship and emotional support to patients and families.
• Perform light housekeeping tasks related to patient care.
• Document care provided in a timely manner and follow individualized care plans.
