Job summary

Vitality Hospice & Palliative Care is seeking a compassionate Certified Nursing Assistant (CNA) for a full-time field-based role. You will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during the end-of-life journey.

The role offers typical hours 7:00 AM – 3:00 PM, Monday–Friday, with occasional evenings/weekends and mileage reimbursement.

Qualifications

• Active CNA certification required.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Ability to work independently in the field.
• Strong communication, time-management, and organizational skills.
• Compassionate, dependable, and patient-focused.
• Experience with hospice, palliative care, or chronic illness preferred (not required).

Responsibilities

• Assist patients with mobility, transfers, and ambulation.
• Provide personal care including bathing, grooming, dressing, and feeding.
• Offer companionship and emotional support to patients and families.
• Perform light housekeeping tasks related to patient care.
• Accurately document care provided in a timely manner; follow individualized care plans and hospice standards.
• Support a culture of compassion, respect, and patient-centered care.

Vitality Hospice & Palliative Care

Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing interdisciplinary team.

In this field-based role, you will provide hands‑on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end‑of‑life journey. Schedule & Position Details

• Full-Time, Monday–Friday
• Typical hours: 7:00 AM – 3:00 PM

• Field-based role with local travel
