Vitality Hospice & Palliative Care in Pennsylvania is seeking a full-time Certified Nursing Assistant (CNA) to join our clinician‑owned team. This field‑based role requires travel to patient residences with mileage reimbursement, and typically runs Monday–Friday, 7:00 AM to 3:00 PM.

Under the supervision of an RN Case Manager, you will provide mobility assistance, personal care, companionship, light housekeeping, and accurate care documentation while following care plans.

Qualifications

• Active CNA certification required.
• Valid driver's license, reliable vehicle, and current auto insurance.
• Compassionate, reliable, and self-directed.
• Strong communication and time-management skills.
• Ability to work independently and follow care plans.
• Commitment to serving patients with chronic, serious, or terminal illnesses.

Responsibilities

• Assist with mobility, transfers, and ambulation.
• Provide personal care (bathing, grooming, dressing, feeding).
• Offer companionship and emotional support.
• Perform light housekeeping tasks.
• Document care accurately and timely.
• Support a culture of compassion, quality, and respect.
• Be an integral member of an interdisciplinary team focused on patient-centered care.
