Vitality Hospice & Palliative Care is seeking a compassionate Certified Nursing Assistant (CNA) for a full-time field-based role. You will provide hands-on home care, helping maintain comfort, dignity, and quality of life for patients in their residences with local travel.

Responsibilities include mobility assistance, personal care, companionship, and timely documentation under RN guidance. Mileage reimbursement, benefits, and a supportive team culture are offered.

Qualifications

• Active CNA certification (required)
• Valid driver's license, reliable vehicle, and current auto insurance
• Ability to work independently in the field
• Strong communication, time-management, and organizational skills
• Compassionate, dependable, and patient-focused
• Experience with hospice/palliative care or chronic illness preferred (not required)

Certified Nursing Assistant (CNA) - Full-Time | Field-Based

Vitality Hospice & Palliative Care

Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing interdisciplinary team.

In this field-based role, you will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end-of-life journey. Schedule & Position Details

• Full-Time, Monday-Friday
• Typical hours: 7:00 AM - 3:00 PM
• Occasional evening/weekend overtime available
• Field-based role with local travel
• Mileage reimbursement provided
• Hiring immediately

Key Responsibilities

Under the direction of a Registered Nurse Case Manager, the CNA will:

• Assist patients with mobility, transfers, and ambulation
