Job summary

ascensionrecovery is seeking a Sr. Case Manager to deliver comprehensive case management for individuals with behavioral health and substance use needs, coordinating care across clinical, medical, and community systems.

You will support engagement, stability, and recovery through advocacy, coordination, and follow-up. The role involves biopsychosocial assessments, treatment plan updates, crisis coordination, discharge planning, and collaboration with community partners.

Qualifications

• Master's degree in social work, psychology, counseling, or related field or three (3) years of behavioral health case management experience.
• Working knowledge of ASAM Criteria, Stages of Change, and recovery-oriented principles.
• Proficiency in EHR documentation.
• Current CPR/First Aid certification (or ability to obtain within 30 days of hire).

Responsibilities

• Conduct biopsychosocial assessments and contribute to treatment plans.
• Coordinate care across clinical, medical, and recovery support teams.
• Provide case management for clients with moderate to high needs, including crisis coordination and discharge planning.
• Facilitate interdisciplinary case reviews and communicate progress and next steps.
• Establish relationships with community agencies, courts, and referral sources.
• Assist clients in accessing housing, employment, benefits, and recovery supports.
• Maintain timely and accurate documentation in the EHR.
• Participate in performance improvement and compliance initiatives.
• Other duties as assigned.
