Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with computers and MS Office applications.
• 2+ years experience in children's mental health, child welfare or related public sector field.
• Willing and able to travel within Ohio up to 50% of the time; reliable transportation.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Evaluate member needs and determine appropriate care and referrals.
• Collaborate with case managers, Medical Directors, and other health programs to optimize outcomes.
• Coach and support members to engage in informed healthcare decisions.
