Job summary

Qualifications

• 2+ years in family systems, case management, or related areas
• 2+ years with Ohio delivery systems/local networks
• Willing to travel within region 50%+; some travel to New Albany office for trainings

Responsibilities

• Coordinate care plans and monitor progress for referred members
• Collaborate with case managers, supervisors, and Medical Directors
• Escalate quality of care issues through established channels
• Provide coaching and support to empower members in healthcare decisions
• Act as a single point of contact to remediate gaps in care and access
