Job summary

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health or related public services field.
• 2+ years in areas: family systems, case management, child development, or related areas.
• 2+ years with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio and travel up to 50%.
• Willing to work beyond standard hours as needed.

Responsibilities

• Coordinate and manage care plans for members and monitor progress.
• Collaborate with clinicians and family teams to optimize outcomes.
• Identify barriers to care and escalate quality of care issues.
• Provide coaching and information to empower members in decision making.
• Ensure compliance with regulatory and accreditation guidelines.
