Job summary

UnitedHealth Group is seeking a Field Care Coordinator to support a caseload with low-to-medium complexity medical/behavioral needs, including ALTCS or Long Term Care services. You will visit members in homes or facilities and collaborate with care teams to improve outcomes.

Required: 2+ years in case management for SMI, 1+ year with elderly or disabled, MS Office proficiency, and valid driver’s license with travel up to 50%.

Qualifications

• 2+ years of case management experience with Serious Mental Illness (SMI).
• 1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Driver's license and access to reliable transportation for ~50% travel within territory.

Responsibilities

• Engage members face-to-face and/or telephonically for comprehensive needs assessment.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with care team, providers, and community resources to execute care plans.
• Educate and coach members for self-management and healthy lifestyle changes.
• Support discharge planning and coordinate care transitions after ER/inpatient/SNF events.
• Advocate for members to ensure needs and choices are represented by the team.
