Job summary

UnitedHealth Group is seeking a field-based Care Coordinator to assess, plan, and implement individualized care strategies, coordinate social services, and manage the care plan across the continuum of care.

About 80% of the time will be spent in the field visiting members at home or in long-term care settings. A valid driver’s license, transportation, and willingness to travel are required; strong MS Office skills are preferred.

Qualifications

• Bachelor’s degree or higher with 2+ years of relevant experience; or 6+ years of related social service or case management experience without a degree.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Valid driver’s license with reliable transportation and ability to travel within the territory to meet members and providers.

Responsibilities

• Assess, plan, and implement individualized care strategies across patient needs.
• Identify and initiate referrals for social service programs including financial, psychosocial, community, and state support services.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate required health-related information to all stakeholders to ensure timely, quality coordinated care.
• Advocate for patients and families to ensure needs and choices are represented by the health care team.
