Job summary

SHELTER, Inc. is seeking a Case Manager for Permanent Supportive Housing in Pittsburgh, CA. Manage a caseload of up to 20 participants, conduct intake, assessments, and coordinate care with partners to support stability and housing retention.

Compensation: $28-30/hour

About the role

We are searching for a Case Manager for our Permanent Supportive Housing programs serving individuals and families experiencing homelessness! The expected case load is up to about 20 participants. This role includes frequent travel for home visits. If you’re a compassionate person who wants to make a life‑changing difference in someone's life, we’d love to talk with you.

What you’ll do:
Intake and Assessment
  • Screen prospective participants by verifying income and documentation, obtaining necessary consents, and reviewing eligibility with supervisors.
  • Conduct comprehensive assessments (HMIS, VI-SPDAT, Housing Retention) to evaluate participant strengths and risks while providing full orientation on program services, rights, and responsibilities.
Case Planning and Monitoring
  • Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans.
  • Establish participant-driven goals based on resource availability, consensus, and required level of care.
  • Provide financial coaching—including budgeting, tax filing (EITC), and credit repair—to support long‑term stability.
  • Link participants to internal SHELTER, Inc. programs and external community resources, ensuring minor children are enrolled in school.
  • Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants.
  • Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations.
  • Manage case closures and exits—both voluntary and involuntary—ensuring all agency procedures and adverse action protocols are followed.
Data and Documentation
  • Keep all relevant service data up‑to‑date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction.
What you’ll bring:
  • Bachelors degree in social services or related area, certification from a four year program in related area, and/or a combination of education and experience that demonstrates a capacity to help people coming from crisis situations and traumatizing circumstances to develop self‑sustainability. Associates degree with relevent experience will be considered.
  • Experience with vulnerable populations, including domestic violence survivors, is preferred.
  • Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel)
  • Ability and willingness to occasionally work flexible hours to accommodate participants available during the evening or on weekends , as agency needs.
  • Experience working in diverse settings with people across all socio‑economic spectrums and a wide variety of personalities and roles - staff, residents, local agencies, contractors, lenders, etc.
  • Billingual is a bonus!
Position Requirements:
  • This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law.
  • Must successfully pass a criminal background check
  • Must provide TB test clearance
