Job summary

Qualifications

  • Bachelor’s degree or equivalent experience in social services required.
  • 1 year of case management experience is needed.
  • Valid driver’s license and auto insurance are mandatory.

Responsibilities

  • Manage a case load of up to 25 individual residents or families.
  • Develop service plans and goals for clients.
  • Assist clients in processing housing applications and lease agreements.
  • Conduct monthly home visits and community visits.

Skills

Experience providing case management services
Experience working with adults with serious mental illness
Excellent written and oral communication skills
Strong interpersonal and organizational skills
Strong computer proficiency (Microsoft Office Suite)

Education

Bachelor’s degree in social work, psychology, sociology, counseling, or related field
