Job summary

Felton Institute's Knox Supportive Services, Senior Division, seeks a Case Manager to coordinate care for adults housed at The Knox. You will work with an interdisciplinary team to provide intensive case management, crisis intervention, and resource connections to support stable housing.

The role requires at least 2 years’ experience with at-risk populations, a focus on unhoused individuals, and strong communication skills.

Qualifications

• Minimum 2 years of case management for at-risk populations.
• Experience with unhoused or formerly unhoused individuals and housing needs.
• Crisis response, de-escalation, and risk mitigation experience.
• Strong communication and engagement with diverse clients.
• Excellent organization and ability to prioritize in field/office settings.

Responsibilities

• Work closely with interdisciplinary team.
• Communicate effectively with internal and partnering agencies.
• Perform intakes and service plan development for adults with housing needs.
• Provide case management services including housing stabilization and resource coordination.
• Maintain detailed client records in accordance with standards.
• Participate in team meetings and case conferences.
• Provide crisis intervention to prevent hospitalization; respond to emergencies.
• Address specific housing needs through training and referrals.
• Commit to ongoing training and evidence-based practices.
• Use ONE system to maintain client housing records.
