Qualifications

• Strong relationship-building with clients and teams.
• Clear verbal and written communication.
• Excellent organizational and time-management skills.
• Ability to work independently and in multidisciplinary teams.
• Bilingual/multilingual skills as applicable.

Responsibilities

• Develop and maintain individualized service plans reflecting client goals.
• Coordinate housing, health, and social services for clients.
• Assist with public benefits applications and renewals.
• Support clients in securing/maintaining permanent housing.
• Deliver crisis intervention and community-based visits.
