Job summary

The Center for Urban Community Services (CUCS) in New York City is seeking an Early AM Case Manager for the Street Outreach Program. Based in Manhattan, you will work with an assigned group of clients to obtain housing, support community integration, and help the team meet housing placement goals.

The shift runs 5:30am–1:30pm Monday–Friday, with occasional four 12am–5am quarterly counts. Responsibilities include assessments, service plans, coordination of housing, health care, benefits, and

Qualifications

• Case Manager I: High School Diploma and 2 years of relevant work experience.
• Case Manager II: Bachelor's Degree; or High School Diploma with 4 years of relevant work experience.
• Case Manager III: Bachelor's Degree and 2 years of relevant work experience; or BSW and 1 year; or HS diploma with 6 years.
• Case Manager IV: Bachelor's Degree and 4 years of relevant work experience; or BSW and 3 years; or HS diploma with 8 years.
• Case Manager V: Internal Only.
• For applicants without a college degree, every 30 college credits may substitute for one year of relevant work experience.

Responsibilities

• Develop and maintain individualized service plans reflecting client strengths, goals, and needs.
• Conduct ongoing assessments and monitor client progress to support stability and recovery.
• Coordinate housing, healthcare, behavioral health, substance use, employment, education, finances, and social services.
• Assist clients with obtaining public benefits by completing applications and coordinating renewals.
• Support clients in securing and maintaining permanent housing, including searches and move-in coordination.
• Provide crisis intervention and coordinate with hospitals, shelters, and other providers.
• Deliver services onsite and in the community, including home visits and appointments.
• Facilitate individual and group services, including psychoeducational groups and wellness programming.
• Maintain documentation and ensure compliance with policies and funder requirements.
