Bay State Community Services (BSCS) is seeking a compassionate Case Manager for its Community Support Program (CSP). The role involves working with individuals facing behavioral health challenges, housing instability, and other barriers to community living.

You will provide strength-based, person-centered case management, coordinate care with families and providers, and connect participants to resources to promote stability and independence.

Formación

• One year of experience with mental health/substance use disorders with a bachelor’s degree in a social services field.
• Or two years of experience with mental health/substance use disorders with Associate degree or High School Diploma
• Deep understanding of systems of care for mental health and SUD.
• Knowledge of Social Determinants/Drivers of Health (SDOH) resources and advocacy.

Responsabilidades

• Provide strength-based, person-centered case management.
• Coordinate care with participants, families, and providers to manage services.
• Serve as contact point, advocate and information resource for participants and care team.
• Plan and monitor care; provide referrals when needed; update plans quarterly.
• Develop individualized service plans and monitor progress; update crisis/safety plans.
