Qualifications

Responsibilities

• Assesses the consumer’s needs and sets goals.
• Identifies goals and plans for services utilizing the ISP.
• Provides linkage to community resources.
• Maintains placement in the least restrictive environment.
• Monitors progress toward goals and documents outcomes.
• Advocates to secure community resources.
