Job summary

AltaPointe Health Systems is seeking a Clinical Case Manager to coordinate Stepping Up program services and conduct comprehensive case management for participants with mental health and co-occurring disorders. You will collaborate with jail staff and local hospitals to implement assessment tools, develop goals, and link clients to community resources.

The role emphasizes advocacy, timely documentation, adherence to AltaPointe standards, and active involvement in quality improvement activities.

Qualifications

• Bachelor’s degree in a mental health discipline and certification of case management training.
• Knowledge in adult specialty to be attained within six months.
• Knowledge of psychiatric and substance use disorders in adults.
• Knowledge of cultural diversity and behavioral management.
• Knowledge of crisis intervention and non-violent crisis practices.
• Knowledge of Psychiatric Rehabilitation and substance abuse treatment principles.
• Knowledge of available community resources and entitlement programs.

Responsibilities

• Coordinates and provides services related to the Stepping Up program.
• Conducts case management services and develops needs assessments and case management plans.
• Represents client needs and ensures quality services within the Stepping Up framework.
• Completes needs assessments and reviews client assets, deficits, and basic needs.
• Develops and implements case management service plans, including goals and resource links.
• Monitors client progress and assists with securing needed services.
• Liaises with community resources, identifies service gaps, and advocates for clients.
• Participates in training and quality assurance activities to maintain standards.
