CVS Health is hiring a Population Health/Wellness Case Management Coordinator for a telework role serving the Kentucky Salt River Trail region. You will support case management, advocate for members and coordinate care using appropriate tools and resources, with travel up to 50% and occasional meetings in Louisville.

Minimum qualifications include a bachelor’s in behavioral health or related field and 2 years in behavioral health or social services; strong computer and MS Office skills are

Qualifications

• Experience in Foster Care and juvenile justice systems, Adoption Assistance, behavior health services and trauma-informed care.
• 2 years’ experience in behavioral health or related field.
• Computer literacy including MS Office Suite (Word, Excel).
• Flexibility to work beyond core hours (evenings/weekends) as needed.

Responsibilities

• Support case management to improve healthcare outcomes for members.
• Advocate, coordinate, educate and support members through care management tools and resources.
• Collaborate with care team to address barriers and facilitate access to services.
• Assist in monitoring and documenting member progress and plan compliance.
