VIVA USA Inc. seeks a Case Management Coordinator to assess, plan, implement, and coordinate case management activities for members to evaluate medical needs and enhance overall wellness.

Role requires case management experience and 50-75% travel for face-to-face visits. Candidates should hold a Bachelor’s degree in social work or related field and have strong Microsoft Office skills including Excel, Teams, and Outlook, with LTC experience preferred.

Qualifications

• Case management experience required.
• Long Term Care experience preferred.
• Microsoft Office experience required: Excel, Teams, Outlook.

Responsabilités

• Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
• Utilize care management tools to plan and monitor care plans.
• Collaborate with PCP and skilled providers to ensure appropriate services.
• Provide coaching and support to empower member decisions and healthy outcomes.
