Job summary

OneWell Health Care in New Hope, PA seeks a caring Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings.

You will assist with daily living activities, meal prep, health monitoring, and community participation while documenting care, following ISP/IPP, and collaborating with the team; flexible schedule and field-based work.

Qualifications

• High school diploma or equivalent.
• Preferred: Prior experience in caregiving, home health, or direct support for individuals with IDD.
• Experience working in community-based or residential support settings.
• Languages: English proficiency required; bilingual skills are a plus.

Responsibilities

• Provide hands-on, person-centered support per care plan.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Monitor health, safety, and emotional well-being; promote independence.
• Maintain structured routines and support community participation.
• Document accurately and timely; participate in trainings and team meetings.
• Support transportation to appointments and community outings.
