Job summary

Molina Healthcare is seeking a care coordination professional to support care management activities and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes and contribute to cost-effective, high-quality care.

Responsibilities include assessments, care planning, and ongoing monitoring, with travel as required. This role calls for proactive, independent work and strong communication skills across provider networks.

Qualifications

• At least 2 years experience in health care or care management.
• Clinical licensure/certification required only if state contract requires it.
• Valid and unrestricted driver's license and reliable transportation for travel.
• Knowledge of community resources.
• Ability to be proactive and detail-oriented.
• Ability to work across diverse settings and with various populations.
• Ability to work independently with minimal supervision.
• Responsive communication and calm under pressure.
• Professional relationship building with colleagues and members.
• Excellent time-management and prioritization skills.
• Strong problem-solving and critical-thinking abilities.
• Strong verbal and written communication skills.
• Microsoft Office proficiency.
• In some states, a health-care related bachelor's degree may be required.

Responsibilities

• Completes member assessments per timelines to determine care coordination eligibility.
• Develops and implements care plans with members, families, physicians, and others.
• Conducts telephonic, in-person, or home visits as needed.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health, LTSS, and home resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines.
• Assesses barriers to care and provides assistance to address concerns.
• Collaborates with licensed care managers as needed.
• May require 25–40% local travel.
