Job summary

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years experience with Ohio delivery systems and MS Office applications.
• Willing to travel within the assigned region up to 50% of the time.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate referred members' needs and eligibility and propose care plans.
• Collaborate with teams to improve quality of care and member engagement.
• Serve as single point of contact to remediate gaps in care and access.
