Job summary

You will provide strength-based and needs-based services, document care, and collaborate with an interdisciplinary team. A CA driver's license and 4 years in mental health/social services are required.

Qualifications

• High School Diploma required with 4 years paid work in mental health/social services (one year in customer service allowed).
• Valid California driver's license, personal vehicle, and proof of insurance; DMV printout required.

Responsibilities

• Develop and deliver a full complement of services: individual, family, group counseling, rehab and case management for assigned caseload.
• Utilize Evidence Based Practice to ensure high quality results and impact.
• Assess risk and intervene based on symptoms, substance use, environment, and physical conditions.
• Create service plans to promote recovery and client-centered goals.
• Engage with outreach and determine disenrollment per program protocols.
• Conduct care planning and therapeutic case management with a flexible approach.
• Document and maintain all data, reporting, and charting in compliance with policies; meet monthly service unit targets.
• Participate in supervision, staff meetings, travel, weekend coverage, and trainings.
• Work independently and as part of a therapeutic team.
• Adapt to new information and changing conditions to meet client needs.
• Balance priorities to respond to changing client demands and improve services.
