Job summary

Bay Area Community Services is seeking a Care Coordinator- Outreach to deliver high-quality therapeutic interventions, case management, crisis support, housing navigation, and family services in Alameda County. This field-based, full-time role requires travel across Oakland, Berkeley, Fremont and surrounding areas, with a Monday–Friday schedule and a focus on client-centered recovery.

Starting pay is $27 per hour, rising to $30 after the 90-day introductory period, plus comprehensive benefits

Qualifications

• High School Diploma required with 4 years of paid work experience in mental health or social services.
• Valid California driver's license, personal vehicle, and proof of personal vehicle insurance.
• DMV printout required.
• Extensive local travel is required.

Responsibilities

• Develop and deliver a full complement of services, including counseling, rehabilitation and case management.
• Use Evidence Based Practice to ensure high quality results and impact.
• Assess risk and intervene appropriately based on symptoms, substance use, environment, and physical conditions.
• Create service plans that are client-centered and goal-oriented.
• Maintain data collection, reporting and charting per policy and regulatory requirements.
