Bay Area Community Services seeks a Care Coordinator/Case Manager to provide face-to-face therapeutic services to chronically homeless individuals across Alameda County. You will deliver counseling, case management, housing navigation, crisis intervention, and family support, traveling as needed and using evidence-based practices.

The role requires driving a personal vehicle, meeting goals, and working as part of a therapeutic team with opportunities for growth within a respected non-profit.

Qualifications

• High School Diploma required and 4 years of paid work experience in mental health/social services. One of those years may be in a customer service function.
• Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage are required. DMV printout required.

Responsabilités

• Development and delivery of a full complement of services, including counseling and case management.
• Utilize Evidence Based Practice to ensure high quality results and impact.
• Continuously assess risk status and intervene appropriately.
• Create a person-centered service plan and monitor progress toward goals.
• Maintain data, reporting, and charting records per policies; provide 65% face-to-face time.
