Job summary

Qualifications

• 1-2 years of related experience
• Bachelor's degree in social science/health-related field preferred or equivalent experience in care coordination for developmentally or intellectually disabled clients or behavioral health clients.

Responsibilities

• Outreach to members via phone or home visits to review care plans and discuss next steps.
• Coordinate care activities with providers, partners, and family/c caregiver to adjust plans as needed.
• Serve as point of contact for member and provider inquiries related to care plans.
• Communicate with care managers and practitioners to ensure continuity of care and services.
• Assist with service assessments or screenings and document member needs.
• Document and maintain member records per state regulations and distribute to providers as needed.
• Follow standards of practice and regulatory guidelines; adhere to policies.
• Identify needs and make referrals to Care Manager or community resources.
