Job summary

Central Ohio Area Agency on Aging seeks a Member Support Coordinator in Columbus to perform assessments, home visits, and follow-up calls, collecting information in the field and reporting to a licensed Care Coordinator who retains clinical authority.

This role requires a 4-year degree, 2 years of human services experience, and a valid driver’s license; it is day shift with potential telework after training.

Qualifications

  • Bachelor's degree required in Social Work, Human Services, Psychology, Sociology, Gerontology, Public Health, Healthcare Administration, or related field.
  • 2 years of experience in a human services, healthcare, or community based support role.
  • Valid driver's license with reliable transportation and proof of insurance.

Responsibilities

  • Conduct in home visits and event based visits as assigned.
  • Complete assigned components of comprehensive assessments and provide results to the licensed Care Coordinator.
  • Observe and document client status and home environment; report observations to the Care Coordinator.
  • Conduct and track check-in calls and follow up contacts per contract schedule and relay updates.
  • Assist in coordinating logistics for services already authorized, such as transportation and meal deliveries.
  • Identify and escalate any signs of changing condition or crisis to staff.
  • Help clients understand scheduled services and direct questions to the Care Coordinator.
  • Maintain accurate and timely documentation of visits and observations.
  • Participate in team meetings, training, and professional development opportunities.
