Job summary

CVS Health in Ohio is hiring a Field-based Case Management Coordinator for the OhioRISE program. The role requires traveling to member homes and locations within Northeast Ohio, roughly half the time, with preference for candidates in Seneca, Lorain, Medina, Wyandot or Crawford counties.

Typical hours are Monday–Friday, 8–5 with flexibility; bachelor’s or master’s in behavioral health or related field is required. Strong coordination and communication skills are essential.

Qualifications

• 2+ years in behavioral health, social/human services
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint)
• 2+ years in children’s mental health, child welfare, or related public sector field
• 2+ years in family systems, case management, or related areas
• Willing to travel within the region ~50%+ of the time
• Reliable transportation; mileage reimbursed
• Ability to work beyond core hours as needed

Responsibilities

• Perform care coordination and education for members
• Coordinate and monitor care plans and progress
• Collaborate with clinical team and other health programs
• Act as single point of contact for members and resolve gaps in care
