CVS Health is seeking a Case Management Coordinator in Ohio to support OhioRISE with care coordination and member education. You will work as part of a multidisciplinary team to help children and families navigate services, assess needs, and implement care plans while traveling within Western Ohio.

The role requires 2+ years in behavioral health, strong knowledge of case management, and the ability to collaborate with providers and families.

Formación

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, developmental disabilities, or related fields.
• 2+ years experience in one or more: family systems, community resources, case management, or child development.

Responsabilidades

• Coordinate and implement assigned care plan activities and monitor progress.
• Evaluate member needs/eligibility and refer to appropriate services.
• Collaborate with care teams to ensure quality of care and timely interventions.
• Provide coaching and information to empower members in healthcare decisions.
